1. LPN & EMT JOB DESCRIPTION
ROLE
-
Assist in Managing Chronic Care Disease Treatments
ESSENTIAL RESPONSIBILITIES
-
Will report to the site's local Medical Director and Nurse Practitioner
-
Assist in the assessment and evaluation of current Health Status
-
Implementation of Diabetic Treatment Plan
-
Ability to perform Vena Puncture
-
Initiating intravenous lines and tubing
-
Administering medication and IV Fluid Therapy
-
Monitoring patient for adverse reactions and ensure that tubing, bandages and needles stay in place
-
Charting Notes
-
Must possess good interpersonal skills
MINIMUM QUALIFICATIONS
-
Graduate of an accredited school, LPN or EMT License
-
Current provider card in Basic Life Support (BLS)
2. REGISTERED NURSE JOB DESCRIPTION
ROLE
-
Responsible for Managing Chronic Care Disease Treatments

Assessment of Health Status
-
Coordinating health care for the diabetic patients in accordance with State and Federal rules and regulations and the nursing standards of care
-
Implementation of plan of care and treatment
ESSENTIAL RESPONSIBILITIES
-
Will report to the site’s on-site Nurse Practitioner or Physician Assistant
-
Will provide Infusion Therapy oversight to LPNs on site and will assist if needed
-
Chairside education should consist of but not limited to:
-
Assists in monitoring the effectiveness of Insulin Resistance Reversal Treatments.

Assists in implementing interventions to support the patient to regain or maintain physiologic stability
-
Document Notes in EMR to be overseen and signed off by the Nurse Practitioner, Physician Assistant, and/or Medical Director

MINIMUM QUALIFICATIONS

Graduate of an accredited school of nursing

Current provider card in Basic Life Support (BLS)

Strong computer skills, including Word, Excel, and PowerPoint
​
EXPERIENCE
Minimum of 2 years’ experience as a Registered Nurse preferred. Any chronic care management is an additional bonus but not a requirement.
